Submit Your Physical Form

Submitting Completed Physical Forms

Athletes must have a current sports qualifying physical exam on file in the Athletic Office before the first day of practice.  Physicals are good for 3 years from the exact date of the last physical.  Download a Physical Form HERE.

Once you have your physical completed by a doctor, you can turn it in to the athletics office or you can scan it and submit the completed Physical Forms online.

After you have completed and submitted the physical form, if you plan on bringing it into the athletic office, or if your physical is current, please click HERE for Step 2: MSHSL Eligibility Form.
